Telemedicine Consent.
Cupid Health, is a Tele-health company that provides a network of providers, nurses, and support teams with state of the art telemedicine technology to lower the cost of patient care in the US. Cupid Health’s providers are licensed in all 50 states. The tele-health services are provided directly to patients or in partnership with hospitals, insurance companies, large employers or other medical companies. Cupid Health’s platform is HIPAA complaint and can integrate quickly into the business operations and technological infrastructure of other medical practices.
Telemedicine involves using technology to enable healthcare providers to evaluate and treat patients from different locations. Telemedicine communications may include transmission of your medical records, photos, and other personal health information, and may include:
- Dynamic intake forms where you enter information electronically via our platform;
- Store-and-forward (also known as “asynchronous) technology, which involves the electronic transmission of your medical information and records, such as medical history, doctor’s notes, diagnostic tests, documents, photos, and videos;
- Secure messaging, including email and text messages;
- Real-time chat and telephone; and
- Live 2-way video and audio.

- Network and software security protocols to protect the privacy and security of your health information.
Telemedicine Makes Life Easier
- Easier access to medical care and treatment,
- Easier access to and from locations that are convenient to you!
- Easier visits as you do not need to be physically present for an in-office appointment.
- And it reduces healthcare costs! Telemedicine may lower medical costs by having online visits and prescriptions.
Telemedicine Also Comes With Some Risks
Please read these risks carefully, telemedicine may not be intended for everyone.
- Telemedicine is not intended to replace a relationship your primary care physician
- Your health care providers may not receive enough information to enable diagnosis or prescription treatments.
- We may not be able to conduct certain tests such as:
- blood work
- assess vital signs viewed in-person,
- Limited access to this information could prevent your provider from identifying a need for emergency medical care or treatment, or underlying causes of any condition for which you are seeking treatment.
- Technical failures may cause delays in diagnosis or treatment
- Telemedicine is not the appropriate course of action in certain medical scenarios.
- In immediate medical situations or allergic or adverse reactions, telemedicine may not be right for you
In immediate and emergency medical situations, please contact your primary care provider or go to your local hospital. If the situation is a medical emergency, please dial 911 immediately.
- Security protocols or safeguards could fail to cause a breach of privacy and unintentional disclosure of your health information.
- State laws may prohibit telemedicine or treatment options, including that you may not be able to receive prescription medications via telemedicine if you reside in certain states.
Your Consent To Telemedicine Services
By Clicking “I Consent” or by using the Websites or the Services, you confirm that you have read the prior section, and you understand the risks and benefits of telemedicine, and you acknowledge and agree:
- You consent to receive telemedicine services from healthcare providers under The Cupid Health.
- You consent to that you authorize health care providers with whom you are connected via the platform, to share information regarding telemedicine services for treatment, payment, and health care operations purposes, including consent to share medical records with your primary care or other physicians.
- You consent to that if you receive a prescription from a healthcare provider, you authorize Cupid Health to send your healthcare and personal information to your dispensing physician or a compounding pharmacy so that you may receive your prescription and other pharmacy services.
- You consent that telemedicine may provide benefits for you, however, no benefits or specific results can be guaranteed
- You consent that your condition may not improve, and in some cases, may get worse.
- You consent to you may need to seek medical care and treatment from your primary care physician or another healthcare provider
- You consent that you have access to all of your health and wellness information relating to telemedicine services in accordance with applicable laws and regulations.
- You consent that telemedicine is constantly changing and growing, and may include different technology and laws that govern it in the future that is not specifically described in this consent.
- You consent that In the event of immediate need for the care you will seek emergency help when needed, and continue to consult with your primary care physician.
You can withhold or withdraw this consent at any time by emailing us at Support@Cupid.health. Unless you revoke your consent, this informed consent will be renewed upon each telemedicine encounter, and your healthcare provider may continue to provide services to you via telemedicine without the need for you to complete another consent.
I understand that telemedicine is the use of electronic information and communication technologies by a health care provider provided by Cupid Health; and hereby consent to those physicians to provide health care services to me via telemedicine.
I understand that the laws that protect the privacy and the confidentiality of medical information also apply to telemedicine.
